How a Visit Works
Four steps, and the first one is the whole trick: show up. Everything after that happens at the clinic.

- Bring three thingsPhoto ID, your insurance card, and a list of current medications you're taking.
- Fill out the paperworkDownload the five forms below and bring them filled in, or we'll hand them to you at check-in and you'll do it here.
- We see you, fix it, send you homeMost visits wrap up in well under an hour. Your first prescription is free, filled before you leave.
Walk in any day, 8:00 AM to 6:00 PM. Closed Easter, Thanksgiving, Christmas Day.
What to Bring
Nothing here is a hard requirement. If you arrive without one of them we will still see you; these three just keep check-in quick and your chart accurate.
